SGA Praxis LLC – The ABEL model

Important Notice: The ABEL model is an organizational planning and change management platform. It is not a healthcare provider and does not provide medical services. This notice explains our data handling practices as they relate to HIPAA and general data consent.
Scope of HIPAA Applicability
The Health Insurance Portability and Accountability Act (HIPAA) applies to "covered entities" — healthcare providers, health plans, and healthcare clearinghouses — and their "business associates." SGA Praxis LLC and The ABEL model are not a covered entity under HIPAA in the context of its standard organizational planning services.
However, if your organization is a healthcare provider or health ministry that uses this platform to manage personnel, volunteers, or mission activities, you should be aware of the following data practices.
What Data We Collect
- Name and email address (for authentication and communication)
- Organizational role and affiliation
- User-generated planning content (goals, tasks, notes, strategic plans)
- File uploads provided by users
- Application usage and activity logs
We do not intentionally collect Protected Health Information (PHI) such as medical records, diagnoses, treatment histories, insurance data, or other clinical health information. Users should not enter PHI into this platform.
What We Do NOT Collect
- Medical records or clinical notes
- Health insurance identification numbers
- Diagnoses, prescriptions, or treatment histories
- Social Security Numbers
- Biometric data
Data Security Measures
We implement the following security practices to protect your data:
- Encrypted data transmission using HTTPS/TLS
- Secure cloud storage infrastructure
- Role-based access controls limiting data exposure
- Authentication via verified providers (email, Google OAuth)
- Regular security monitoring and updates
Consent to Data Processing
By creating an account and using The ABEL model, you consent to the following:
- Collection and processing of your account and organizational data as described in our Privacy Policy
- Use of anonymized usage data to improve the platform
- Storage of your data in secure cloud infrastructure
- Communication from SGA Praxis LLC regarding your account and the platform
- Use of AI tools to assist with planning and content generation based on the data you provide
Your Rights
Regardless of HIPAA applicability, you have the right to:
- Request access to your personal data
- Request correction of inaccurate data
- Request deletion of your account and associated data
- Withdraw consent to data processing (resulting in account termination)
- File a complaint with us or applicable data protection authorities

Healthcare Organizations Using This Platform
If you are a healthcare provider, health ministry, or organization subject to HIPAA and you wish to use The ABEL model for managing health-related personnel or operations, please contact us before proceeding. We may require a Business Associate Agreement (BAA) and additional safeguards. Do not enter any PHI into the platform without first establishing appropriate agreements with SGA Praxis LLC.
Breach Notification
In the event of a data security incident that may affect your personal information, SGA Praxis LLC will notify affected users in a timely manner consistent with applicable law. Notifications will be sent to the email address associated with your account.
